MOT failure on cyclic revving

My '97 VR6 failed today. It was unable to complete the emissions test because on the fast-idle part, the engine decided to cyclically rev between around 1500 and 2000. This stopped above 2000, but any attempt to hold the revs in that sort of range (1500) just resulted in this rhythmic cycling (Surging at about a second interval). I have noticed a slight dead spot at that point in acceleration during normal driving, where the power dips slightly and then picks up. Any ideas what this might be?

Also how do you adjust headlight height? Doesn't seem too obvious.
